We are excited to welcome new patients to Strong Roots Family Clinic. Our goal is to make the process straightforward and comfortable from the beginning.

At opening, Strong Roots Family Clinic will operate as a cash-pay clinic. We are currently welcoming self-pay patients while insurance enrollment, credentialing, and payer contracts are still in process. As insurance contracts are completed, we plan to add participating plans and will post updates on the website.

Please bring a photo ID, a list of current medications, relevant medical records if available, and any completed new patient paperwork.

Patients may call the office for current self-pay rates, available services, and scheduling information.
